Resumen : O tabagismo é considerado a principal causa de morte evitável e o maior fator de risco para desenvolvimento de doenças pulmonares e cardiovasculares. A leptina inibe a liberação de cortisol podendo aumentar a fissura pelo cigarro, dificultando o processo de cessação tabágica. Atualmente, são escassas na literatura, informações sobre a influência da leptina e do cortisol no processo inicial de cessação tabágica e sua relação com a fissura, tornando-se clara a necessidade de mais informações para elucidar tais conhecimentos. A presente dissertação está apresentada na forma de um artigo original que contempla o seguinte objetivo: avaliar a influência da concentração sérica de leptina e cortisol de fumantes no início do tratamento sobre a fissura e status tabágico após um mês de intervenção para a cessação. Trata-se de um estudo de intervenção, realizado com tabagistas em tratamento para a cessação tabágica. Antes de iniciar a intervenção, os tabagistas realizaram coleta de sangue em jejum, para análise das concentrações séricas de leptina e cortisol. A avaliação antropométrica foi realizada no início e após um mês de tratamento. A concentração de leptina foi ajustada, por meio da técnica de regressão linear, pelo Índice de Massa Corporal Inicial (IMC) (leptina/IMC), e pelo Percentual de Gordura Corporal Inicial (%GC) (leptina/%GC). O grau de fissura foi avaliado através do Questionnaire of Smoking Urges-Brief (QSUBrief). O QSU-Brief foi avaliado quanto ao seu somatório e pontuações do fator 1 e fator 2. As análises estatísticas foram realizadas no programa estatístico PASW 17.0. Dos 67 voluntários avaliados, 46 completaram o acompanhamento de 1 mês. Daqueles que concluíram o tratamento, 24 permaneceram fumantes e 22 atingiram a abstinência. Entre os fumantes, a média e desvio padrão de leptina/IMC e leptina/%GC foi respectivamente de 11,91±5,82 e 12,12±6,89 ng/mL. Entre os que conseguiram se abster, a média foi 11,32±5,47 e 11,36±6,54 ng/mL. Já o cortisol apresentou entre os fumantes, mediana e intervalo interquartil de 8,95 (8,20-19,37) e entre os abstinentes 11,00 (7,40-14,40). Não houve diferença nas concentrações iniciais de leptina e cortisol entre os grupos (p> 0,05). Houve correlação entre o QSU-Brief (Fator 1 e 2) inicial com a concentração de leptina/%GC entre os que continuaram fumando (QSU-Brief: r=0,497; p=0,014; Fator 1: r=0,434; p=0,034; Fator 2: r=0,442; p=0,030). Após 1 mês, houve correlação entre leptina/%GC e o QSU-Brief (Fator 2), nos fumantes (QSU-Brief: r=0,464; p=0,026; Fator 2: (r=0,595; p=0,003). Já entre os abstinentes, houve correlação apenas entre leptina/IMC e o QSU-Brief (Fator 2) inicial (r=0,489; p=0,021). Os indivíduos que apresentaram maiores concentrações de leptina/%GC (> 12ng/mL) apresentaram maior pontuação do Fator 2 do QSU-Brief após 1 mês de tratamento (n=15; p=0,030). Foi observada correlação entre leptina/IMC e leptina/%GC com o QSU-Brief (Fator 2) de 1 mês entre as mulheres que permaneceram fumantes (r=0,565; p=0,023) e com o QSU-Brief (Fator 2) inicial entre as mulheres abstinentes (r=0,551; p=0,033). Já entre os homens, houve correlação apenas entre os níveis de cortisol com o número de cigarros/dia após 1 mês entre aqueles que continuaram fumando (r=-0,587; p= 0,035). Conclusão: As maiores concentrações de leptina foram associadas com maior fissura e dificuldade em conseguir a abstinência. Houve correlação positiva da leptina/IMC e leptina/%GC com o reforço negativo QSU-Brief (Fator 2), após 1 mês nas mulheres que não conseguiram parar de fumar.
metadata.dc.description.abstracten: Smoking is considered the leading cause of preventable death and the largest risk factor for developing lung and cardiovascular diseases. Leptin inhibits the release of cortisol and can increase the cigarette craving, hindering the smoking cessation process. Currently are scarce in the literature, information about the influence of leptin and cortisol in the initial process of smoking cessation and its relationship with the craving becoming clear the need for more information to elucidate this knowledge. This work is presented as an original article that includes the following objective: evaluate the influence of serum leptin and cortisol of smokers early in treatment on craving and smoking status after a month of intervention for the cessation. This is an intervention study, conducted with smokers in treatment for smoking cessation. Before beginning the intervention, smokers performed blood collection in the fasting for analysis of serum leptin and cortisol. The anthropometric measurements at baseline and after one month of treatment was performed. The leptin concentration was adjusted by linear regression technique, by initial Body Mass Index (BMI) (leptin/BMI), and the initial Body Fat Percentage (%BF) (leptin/%GC). The degree of craving was evaluated using the Questionnaire of Smoking Urges-Brief (QSU-Brief). The QSU-Brief was evaluated for sum scores and Factors 1 and 2. The statistical analyzes were performed using the statistical program SPSS 17.0. Of the 67 individuals evaluated, 46 completed the follow-up of 1 month. Of those who completed the treatment, 24 remained smokers and 22 achieve abstinence. Among smokers, the mean and standard deviation of leptin/BMI and leptin/BF% was respectively 11.91±5.82 and 12.12±6.89 ng/mL. Among those who were able to abstain, the average was 11.32±5.47 and 11.36±6.54 ng/mL. The cortisol presented among smokers, median and interquartile range of 8.95 (8.20-19.37) and among abstinent 11.00 (7.40-14.40). No differences in the initial concentrations of cortisol and leptin between the groups (p> 0.05). There was a correlation between the QSU-Brief initial (Factor 1 and 2) with concentration of leptin/BF% among those who continued smoking (QSU-Brief: r=0.497; p=0.014; Factor 1: r=0.434; p=0.034; Factor 2: r=0.442; p=0.030). After 1 month, there was correlation between leptin/BF% and the QSU-Brief (Factor 2), in smokers (QSU-Brief: r=0.464; p=0.026; Factor 2: (r=0.595; p=0.003). Among abstinent, there was correlation only between leptin/BMI and the QSU-Brief (Factor 2) initial (r=0.489; p=0.021). The individuals who presented higher leptin/%BF concentrations (> 12ng/mL), showed higher scores of QSU-Brief (Factor 2), after 1 month of treatment (n=15; p=0.030). There was a correlation between leptin/BMI and leptin/%BF with QSU-Brief (Factor 2) of 1 month among women who remained smokers (r=0.565; p=0.023) and the QSU-Brief (Factor 2) initial among the abstinent women (r=0.551; p=0.033). Among men, there was correlation only between cortisol levels with the number of cigarettes/day after 1 month among those who continued smoking (r=-0.587; p=0.035). Conclusion: The higher leptinconcentrations were associated with greater craving and difficulty in achieving abstinence. There was a positive correlation between leptin/BMI and leptin/%BF with negative reinforcement QSU-Brief (Factor 2), after 1 month in women who have failed to quit smoking.
