Resumo: Estímulos agradáveis ou desagradáveis causam reações emocionais relacionadas ao sistema motivacional apetitivo ou defensivo, respectivamente. Sabe-se que as reações emocionais aos estímulos podem ser moduladas pelo conhecimento prévio do indivíduo. O objetivo geral do presente estudo foi avaliar as reações emocionais a imagens de esfregaços citológicos de colo do útero, em amostras com diferentes graus de experiência e se essas reações influenciam a precisão diagnóstica. O estudo foi dividido em duas etapas. Na primeira etapa, a amostra foi composta por 146 estudantes da Universidade Federal de Ouro Preto (UFOP) dos cursos de Medicina (N = 82) e Direito (N = 64) (M = 21,3 anos; DP = 3,0) e 34 citopatologistas com pelo menos 2 anos de experiência (M = 45,6 anos; DP = 11,1). Os participantes viram 60 imagens do catálogo do International Affective Picture System (IAPS), sendo 20 negativas, 20 positivas e 20 neutras, e 20 imagens de citologia cervical (10 com alteração e 10 sem alteração). A avaliação emocional das imagens seguiu o protocolo padrão do IAPS e os participantes preencheram a Escala de Valência e Escala de Ativação Emocional utilizando a versão em papel e lápis. Os resultados mostraram que a valência e a ativação para imagens positivas, neutras e negativas do IAPS não foram moduladas pela experiência prévia em citologia. Por outro lado, ambos os grupos de alunos julgaram as imagens citológicas com e sem alteração, como menos agradáveis e menos ativantes do que os citopatologistas. Os alunos julgaram as imagens citológicas com e sem alteração com a mesma valência e ativação. Para os citopatologistas, as imagens com alteração foram menos agradáveis e mais ativantes do que as sem alteração. Na segunda etapa, a amostra foi composta por 22 estudantes do curso de Medicina da UFOP (M = 24,1 anos; DP = 4,1) e 4 citopatologistas com pelo menos 2 anos de experiência (M = 43,0 anos; DP = 11,0). Os participantes viram 20 imagens neutras do catálogo do IAPS, 20 imagens de citologia cervical com alteração e 20 sem alteração. Ao final de cada bloco de imagens, os participantes avaliaram as emoções que estavam sentindo (medo, raiva, repulsa, tristeza, alegria, surpresa, neutralidade, valência e ativação) e ao final do bloco de citologia cervical, avaliaram também a precisão diagnóstica. As imagens com alteração citopatológica promoveram maior medo e menor valência do que as imagens sem alteração e as neutras, independente do grupo. Os citopatologistas avaliaram essas imagens com maior alegria e maior ativação do que os estudantes. Para os estudantes, as imagens com alteração promoveram menor alegria do que as imagens neutras e as sem alteração. Os citopatologistas tiveram maior certeza que as imagens com alteração tinham tumor e maior certeza que as imagens sem alteração não tinham tumor em relação aos estudantes. As emoções discretas influenciaram na precisão diagnóstica. Independente do grupo, quanto maior o medo, a raiva e a repulsa e menor a agradabilidade das imagens sem alteração, maior a certeza de que elas tinham tumor. Quanto maior a ativação, o medo e a surpresa e quanto menor a neutralidade das imagens com alteração, maior a certeza que elas tinham tumor. Concluiu-se que o conhecimento prévio sobre o conteúdo das imagens dos esfregaços citológicos do colo uterino influenciou as respostas emocionais e a precisão diagnóstica, enfatizando o papel das diferenças individuais em relação à experiência na especificidade das reações emocionais.
Resumo em outra língua: Pleasant or unpleasant stimuli cause emotional reactions related to the appetitive or defensive motivation systems, respectively. It is known that the emotional reactions to the stimuli can be modulated by the previous knowledge of an individual. The general objective of the present study was to assess the emotional reactions to the images of cytologic smears of uterine cervix in samples with different levels of experience and whether these reactions do influence the diagnosis accuracy. The study was divided in two stages. In the first stage, the sample was composed of 146 students from the Federal University of Ouro Preto (UFOP), from the courses of Medicine (N = 82) and Law (N = 64) (M = 21.29 years old; DP = 3.04) and 34 cytopathologists with at least 2 years of experience (M = 45.46 years old; DP = 11.13). The participants looked at 60 images from the catalog International Affective Picture System (IAPS), were 20 negative, 20 positive, and 20 neutral, and 20 images of cervical cytology (10 with alteration and 10 without alteration). The emotional assessment of the images followed the standard protocol of IAPS and the participants filled out the Valence Scale and Emotional Activation Scale using the paper and pencil version of the SAM. The results showed that the valence and the activation for positive, neutral, and negative images from the IAPS were not modulated by the previous experience on cytology. On the other hand, both groups of students considered the cytologic images with and without alteration as less pleasant and less activating than the cytopathologists. The students considered the cytologic images with and without alteration as having the same valence and activation. For the group of cytopathologists, the images with alterations were less pleasant and more activating than the ones without alteration. In tem second stage, the sample was composed of 22 students from the Medicine course of UFOP (M = 24.12 years old; DP = 4.06) and 4 cytopathologists with at least 2 years of experience (M = 43 years old; DP = 10.98). The participants looked at 20 neutral images from the IAPS catalog, 20 images of cervical cytology with alteration and 20 without alteration. At the end of each block of images, the participants assessed the emotions that they were feeling (fear, anger, disgust, sadness, happiness, surprise, and neutrality, valence, and activation) and at the end of the block of uterine cervix cytologic slides they also assessed the diagnosis accuracy. The images with cytopathological alteration promoted greater fear and lower valence than the images without alteration and the neutral ones, regardless of the group. The cytopathologists assessed these images with greater happiness and activation than the students. For the students, the images with alteration promoted less happiness than the images without alteration and the neutral ones. The cytopathologists were more certain that the images with alteration had a tumor and more certain that the images without alteration did not have a tumor, when compared to the students. The discrete emotions influenced in the diagnosis accuracy. Regardless of the group, as higher the fear, anger, and disgust and lower the pleasantness of the images without alteration, more certain they were that the images had a tumor. The higher the activation, fear, and surprise, and the lower the neutrality of the images with alteration, the higher the certainty that they had a tumor. It was concluded that the previous knowledge about the content of the images of cytologic smears of uterine cervix influenced the emotional responses and the diagnosis accuracy, highlighting the role of individual differences in relation to the experience in the specificity of emotional reactions.
